Access To Health Care Records and How to File A Complaint

  • Current and former clients may request their personal therapy records from NVM Therapy PLLC by emailing natalia@nvmtherapy.net.

    Records must be requested by either the individual themselves, or via another individual that possesses documentation proving legal authority, such as guardianship or medical power of attorney. Document and identity verification is used to ensure protection of legally mandated client privacy and confidentiality.

  • NVM Therapy PLLC is licensed as a Marriage and Family Therapist and Licensed Professional Counselor in the state of Texas. Both of these licenses are regulated through the Texas Behavioral Health Texas Council.

  • Consumer complaints may be submitted through the Office of Attorney General’s Consumer Protection website: https://www.texasattorneygeneral.gov/consumer-protection/file-consumer-complaint

    Include any relevant names, dates, documentation and licensure information that’s available.
